To Schedule your first appointment:
We are excited to have you as a member! By being structured as a membership practice, we are able to truly help our members feel better and make a difference in our community. Thank you for being a part of this.
It is important to us that we listen. We want you to be heard! Our first appointment will be a “New Patient Appointment”. We set aside 90 minutes so that we can get to know the unique you.
To schedule your New Patient Appointment:

For your New Patient Appointment:
Please arrive 10-15 minutes early for the check in process. Bring all medications and supplements you take.
Lastly, you will receive an email with new patient health history forms to fill out. Please have these forms completed at least the day prior to your appointment. If you don’t see them in your inbox, please check your spam folder.

We are located Centrally in Vancouver at 2102 East McLoughlin BLVD, Vancouver, WA, 98661. We are the corner single story brick building. When you arrive, please check in at the front desk. If no one is at the desk, please ring the bell and someone will be with you shortly.
